Poetry manuscript? What's in a Bio?

I will be submitting my book of poetry to a publisher soon, but I am still not sure on how the manuscript should be put together. They want, between 300 to 500 word synopsis and an author bio included.
Does this apply to poetry works?
They also want a complete copy of the manuscript sent as an attachment in an email.

What should I include in my Bio since I have only been published in Anthologies that were, basicly, rip offs?

Any help would be greatly appreciated.
