Please help me find this poem!!!!

Hi I've been searching for the title and text of this poem for some time. I think it's by Walt Whitmann but I'm not sure. If anybody has seen the movie "Patch Adams", they will recall Patch reciting this poem to his girlfriend. One of the lines reads "...I love you so close that when you close your eyes, I fall asleep...". This poem means so much to me & I really need to know the title & full text. Please please help me!
