help finding a poem "my life story in five short chapters"

Need help finding a poem with title "my life story in five short chapters" or something similar to that. It talk about makes she keep making, falling in a hole and being blind to it, falling in a hole she knew was coming, falling in a hole because she can't help it, then avoid the hole and taking another way. thanks!
