Anyone who is social and lives long enough
will encounter someone whose whole life
and viewpoint from it is determined by their misery;
a pain which has no origin in any kind of actual ailment,
past or present physical or mental abuse by any outside
source, yet is treated and is expected to be treated as the
preeminent consideration.  It is a pain that derives
from not being able to re-create the world in their own image
so that it is in a constant state of awe and adoration for its
benefactor.  It creates an atmosphere and an idiom
however irrational that if one is unfortunate
he will assimilate and give deference to
even at the cost of his own well being.

I have never in my life known such a person who at the same time
had conscience enough to appreciate that what they were doing
to others was wrong.

May you give an example?   It is hard to understand what you are referring to from your descriptions alone.
