here it goes...

why does it hurt so bad
when you finally realize you need to change
why does it dig so deep
when you're ready to let go of the pain
to finally realize you can't do it alone
and to let go of the anger
to let the hurt out
and finally be willing to share
to deal with the fact
that you're not always right and thats ok
to take pride in your true gifts
and go on throughout the day
to accept the fact
that yeah you're hurt and its ok to cry
to stop holding on to the past
and learn to say goodbye
when your bodys run down
from all the running away
when you finally tell yourself
enoughs enough and your going to stay
but why does it hurt so bad
when you've decided to make that change
when reality comes to haunt you
and its like a slap in the face
it hurts like the anger
you've been hiding for so long
you wipe the tears from your eyes
and tell yourself you have to go on
but why does it hurt so much
to admit to people that you need to change
especially the people that you love
to no longer hide the hurt
like old dust under the rug
to let the real you come out and no longer hide
behind something thats not really you
to know that not everybody
is going to like you no matter what you do
so when your legs are growin weak
from all the running you've done
stop and think if its really worth it
all the hurt and anger and pain
you carry like luggage
its all so right and you know
you need to do it and learn to love
but why if its all so right, does it hurt so much?

"to deal with the fact
that you're not always right and thats ok"

That was a line I would love to tell many people... (ok, I needed to hear it too.)  But this is a very well written poem.  I see nothing that needs "ripping apart".  The ending was perfect.  Why do we feel pain, when we know should never have to?
Very nicely written.
I haven't seen much of your work around here, but I look forward to reading more!  Nicely done.
