I never met a woman
Who loved me for who I am
Just cause I was  "doctor"
Could give her more than SPAM.  

But I never loved a woman
Who was out of shape and fat
I've always picked my women
Like one would pick a cat

First there was the dancer
But she left me for "LA"
Then the rock musician
Soon we parted ways

But then I met the nicest girl
Made her life a living hell
Over years I fell in love with her
By then she could not tell

I thought we were built on rock
But it was simply sand
When things got tough and I failed
She found another man

Now I know the rules of love
But I am broken in a pile
This is the price that I pay
For acting like a child

I could never love another girl
But if I could there's one condition
That she simply never knew
I have been called "physician"

Love is elusive at best. It seems the character you describe has mostly experienced the unrequited side of it.
